Tilted Mill was established in 2002 by Chris Beatrice and Peter Haffenreffer, veteran game developers from Impressions Games, known for their historical strategy games. They left Impressions prior to its closure in April 2004.
Tilted Mill continues to focus on the same kind of games, with Immortal Cities: Children of the Nile in 2004, and Caesar IV in 2006. Ironically, many of the employees had been working on the previous City Building series at Impressions. Caesar IV was published by Vivendi Games, the franchise rightsholder at the time and the same company that had closed the Impressions studio in 2004.
In 2007, the studio contributed to the long-running SimCity franchise with SimCity Societies and the expansion pack Destinations.
In 2008, an enhanced re-release of Children of the Nile called Children of the Nile: Enhanced Edition was made available, followed in September by Hinterland, a fantasy strategy game based on completely new IP, and in November of that year Mosby's Confederacy, a strategy game set during the American Civil War.
The development of their game Medieval Mayor was halted on 11 October 2013, and aside from two posts on the official Facebook page (the latest one being from 12 April 2016), the company was not publicly active until 20 May 2025, when Chris Beatrice posted about the company's reactivation on Reddit.
