How a Healthy Hentai Administers Public Service is the follow-up to How to Live a Healthy Hentai Lifestyle!, set in the same universe but starring a different protagonist. Both games take place in a society where sexual fetishes are celebrated instead of shunned and public displays of sexuality are encouraged. At specific points throughout the game, the player can make a choice which will determine where the story goes. It features voice acting for all significant characters except the protagonist.

This story follows a man by the name of Jin Shintani who has just been recruited into the Office of Sexual Affairs, an organization which upholds social order in the city of Vaginville by providing sexual relief to needy members of the public. Jin's coworkers are Hiyori Honma, a perverted young woman who joined the organization at the same time as him and is eager to sate her lust on the job; Taki Kitagawa, an experienced officer in charge of sexual patrols who uses her expertise to quickly get the job done wherever she is needed; Camille Maezono Chabrier and Mireille Chika Chabrier, a pair of near-identical twins whose only difference is what exists between their legs; and Rinko Takasago, a member of an elite team named PUSSI (Peculiar, Unusual, and Specialized Sexual Intelligence) which deals with highly specific fetishes.
