Scarface: The Last Stand is a gallery shooter based on the final shootout starring Tony Montana from the 1983 movie Scarface. The game can be played either in a regular mode or in an endless survival mode, but with similar gameplay. Tony Montana is shown from a behind view in fixed, single screen location. The screen is divided into nine sectors that correspond with the number on the keypad, so pressing 9 will make him fire in the top right sector and 4 the left one in the middle. Different types of enemies appear and need to be taken out. Tony can quickly move from an opening position to cover. Cover prevents him from getting hits by bullets, but not grenades and rockets as these damage the cover and need to be shot out of the air. While in cover the gun's ammo is replenished and some health is restored. Moving from cover to the open firing position and vice versa is the only form of movement that is possible.

During gameplay power-ups sometimes appear. These need to be shot to activate them. There is armor that catches ten bullets without damage, a grenade that offers three rounds for a grenade launcher, rage for improved damage, unlimited ammo for a short period of time, a star for a score bonus, full health, and an explosive that damages everything on the screen. Damage to Tony is shown as bullet holes on the screen. When there are too many he dies. Enemies preferably need to be engaged when there is a blue bar above their heads as that means they are reloading. A score multiplier up to ten for successive kills can be gained.
