Boyfriend to Death 2: Fresh Blood is an erotic horror visual novel and the sequel to Boyfriend to Death. The player takes on the role of the protagonist who decides to go to a cafe, pub, or bar. There, they get tangled in the sadistic plans of any of the four main characters: Lawrence is soft-spoken yet odd, Cain is a chaotic fallen angel, Vincent is uncontrollably aggressive, and Ren is a likeable half-human fox.

Just like its former entry, Boyfriend to Death 2: Fresh Blood combines elements of survival horror and dating simulation, where the player must take on different choices to ensure their own safety. It has an emphasis on gory situations, incorporating several scenes of non-consensual sexual acts, torture and general violence.
