A Plague Tale: Innocence is a single-player stealth-based action game. The story takes place in 14th-century France during the Hundred Years’ War, where the land is devastated by both armed conflict and the spread of a mysterious plague. The player controls Amicia de Rune, a young noble forced to flee her home with her younger brother Hugo after their family is attacked by the Inquisition. Amicia must guide Hugo across villages, battlefields, and ruined countryside, all while uncovering the secret behind his unusual illness and its link to the swarms of rats overwhelming the land.

The gameplay combines stealth, environmental navigation, and light puzzle-solving. Amicia is lightly armed, relying primarily on her sling to distract guards, break objects, and defend herself in limited situations. Combat is not the focus: instead, players are encouraged to avoid direct confrontation by sneaking past patrols, using cover, and manipulating the environment to create opportunities for escape. The sling can be upgraded over time, allowing for more precise shots, faster reloads, and enhanced utility against armored enemies.

A central mechanic involves swarms of plague-bearing rats that shun light but devour anything caught in darkness. Players must solve environmental puzzles that revolve around controlling sources of illumination, such as torches, braziers, or lanterns, in order to carve safe paths through infested areas. Rats can also be used strategically against human enemies, driving them into the darkness or extinguishing their protection to feed the swarm.

Progression is linear, with each chapter introducing new scenarios, locations, and enemy types. Amicia acquires crafting materials that allow her to prepare alchemical tools, such as Ignifer to ignite fires or Devorantis to force enemies to remove helmets. Crafting expands tactical options while reinforcing the scarcity of resources.
