Venus Vacation Prism: Dead or Alive Xtreme is a single-player and multiplayer life and dating simulation game. The story takes place on a private island where the female characters of the Dead or Alive universe are invited to spend a tropical holiday. The player assumes the role of host and organizer, overseeing the vacation activities while interacting with the cast of fighters who now focus on relaxation, competition, and building relationships in a non-combat setting.

The core gameplay revolves around scheduling and managing the vacation while participating in a variety of mini-games. Beach volleyball is the central activity, offering two-on-two matches that emphasize positioning, timing, and coordination rather than direct combat. Matches are played in short sets, and player performance is measured not only by wins but also by how well partners cooperate. Alongside volleyball, the game includes water-based activities such as pool hopping, butt battles, and tug-of-war, each designed as lighthearted competitive diversions.

Progression is tied to developing bonds with the characters. Players choose who to spend time with, influencing affinity levels through successful match participation, gift-giving, and dialogue choices. Costumes and accessories play a key role in the progression system, as characters have preferred styles that impact their mood and willingness to engage.
